Private Insurance and Medicare Patients …For your protection, HIPPA regulations do not permit the transmission of confidential, patient information over our website. Instead, in order to speed up the intake process of your first appointment, we invite you to print out the forms at home, fill them out, and bring them in for your Initial Evaluation.
Otherwise, feel free to come in and fill them out in person, allowing a few extra minutes before your appointment.
